Job Overview:

The Per Diem Case Manager understands the financial responsibilities of the case management department and safeguards system resources by actively participating in utilization management and insurance communication, and collaborates with Social Worker in all discharge planning.

Qualifications:

Required:

• Master s degree in Social Work

Certifications and Licenses Required:

• NJ Licensed Social Worker

Scheduling Requirements:

• Shift working, 8:30-4:30pm

• Monday-Saturday

• PD

Essential Functions:

• Collaborates closely with physician advisor

• Participate in interdisciplinary rounds and facilitate communication with the interdisciplinary care team

• Complete psychosocial assessments and implement patient-centered care plans that meet needs identified in assessments

• Discharge planning activities: seamless and timely coordination of care including: referrals to home care, DME, SNF/ rehab placements hospice, community resources, and transportation arrangements

• Participates in risk management referrals

• Ensures that staff operate within compliance with all applicable policies, laws and regulations

• Collaborate with department leadership to streamline processes and implement strategies to create more efficient delivery of care

• Ensures compliance with regulatory documentation standards
